Step 1: Downloading Zoom

1.  Click on the link you received via email or text

2. Click on Download Now

3. Follow the instructions to install Zoom

On Desktop/Notebook Computer:

• Click on the Zoom installer file
  
• Install Zoom client & app
• Allow Zoom to access your device’s microphone & camera

On iPhone/iPad:

• Confirm Zoom app install from App Store
  
• Install Zoom app
• Open Zoom when finished installing
• Allow Zoom to access your device’s microphone & camera

On Android:

• Confirm Zoom app install from Google Play Store
  
• Install Zoom app
• Open Zoom when finished installing
• Allow Zoom to access your device’s microphone & camera

Step 2: Using Zoom for Your Appointment

1.  Shortly before your appointment, click on the link you received via email or text

2. Zoom will open automatically on your device

3. Allow Zoom to use your microphone

4. Wait for the doctor to begin the meeting

5. Click Start Video once you have joined the meeting

6. When your appointment is finished, click Leave Meeting

Step 3: Your Physician Certification Form

1.  After your appointment, you will receive email & text notifications of your approval*

2. You will receive your completed Physician Certification form within 24 hours

3. Your Physician Certification form will come with instructions on how to Apply For Your Kentucky Medical Marijuana Card using the KY Complia website portal

*patients must meet qualifying conditions as specified per Article XIV, Section 1.2 of the Kentucky Constitution
